Plan Your Healthy Future… And Help Improve a Health Resource, Too [Name of organization] is organizing a health education event called Generate My Healthy Future Plan for YOUth. We will match you to a community volunteer for a structured conversation about what you want for your healthy future. A whole range of subjects are up for exploration. It all depends on what’s on your mind. You’re in charge of what gets discussed and what you keep private. Also, Healthy Teen Network, the developer of Healthy Future Plan, is conducting a study on the activity. And they invite YOUth to take part in the activity and provide them feedback. • • • • •
Youth between ages 14 and 24 are eligible. Parent/guardian permission is required for youth 17 and under. Participation is completely voluntary. Easy post-activity questionnaire for feedback.
