FRANKLIN COUNTY JUNIOR FAIR BOARD 4-H POULTRY LIVESTOCK PROJECT IDENTIFICATION FORM A 4-H member must complete this identification form for ALL poultry projects he/she plans to exhibit/show at the Franklin County Fair. The form must include every bird being cared for by the 4-H member. All birds must have some form of identification. It is the responsibility of the 4-H member’s family to have the animal properly identified with a band or wing tag by possession date. Please refer to the livestock possession information to determine possession date and the required documentation submitted with this form. ** Poultry Requirement** All animals MUST submit a copy of the proof of purchase NIPIP hatchery papers or proof of pullorum free with this Identification Form. (A copy of group hatchery receipt is acceptable). If the animal was not purchased from a certified pullorum free hatchery it is the exhibitor's responsibility to have the animal blood tested no more than 90 days prior to the fair and tested by an approved pullorum tester by the ODA, and certify that the animal is pullorum free prior to the fair. A copy of this certificate must be submitted to the Junior Fair Board prior to the start of the fair. WATER FOWL are exempt from having to show proof of being pullorum free but must send a copy of hatchery papers.
