LifeMap Assurance Company Provides a suite of supplemental insurance coverages to ensure your employees are taken care of. CoPatient Offers members free analysis of health care bills to identify possible errors and overcharges, as well as an optional negotiation service to save members money on health care bills.

Eligibility
AGC Health Benefit Trust is a bona fide construction industry association health plan and must limit group health plan participation based on the following criteria: n SIC Code in the range of 1500–1799 — OR — n CCB license from State of Oregon or State of Washington — OR — n Certification that 50% or more of annual revenue is received from work in the commercial construction industry. 10/14
