LA JOYA ISD Procedures for identifying children and youth facing homeless situations under the McKinney-Vento Act: 1. The clerk at each campus who received the student enrollment packets will pull out the Student Residency Questionnaires collected at the beginning of the year or as students enroll. They will be given to the parent educator or social worker. The parent educator or social worker at each campus will be responsible for identifying potential students who may qualify for services under the McKinney-Vento Act using the Student Residency Questionnaire. 2. The campus parent educator or social worker will use the Interview Script Form, if necessary, with the Student Residency Questionnaire to follow up with an interview that will help clarify the student’s homeless situation and what services he/she may qualify for. 3. The parent educator or social worker will give the Student Residency Questionnaire and the Interview Script Form (if used) to the Administrator in charge of PEIMS to review, assign code(s) and sign the form(s) for those students that have been identified as facing a homeless situation or are an unaccompanied youth. The administrator will then give the completed form(s) to the PEIMS clerk who will enter the proper code for homeless and unaccompanied youth in SkyWeb and file the form(s) in the yellow At-risk Folder found in the student permanent record. The parent educator or social worker is to keep a copy of the Student Residency Questionnaire and the Interview Script Form (if used) as they work with the student facing the homeless situation. The Student Residency Questionnaire in the At-risk Folder can be audited by TEA for up to 5 years. 4. Students who meet eligibility as homeless maintain that eligibility for the remainder of the school year regardless of changes in circumstances relating to parent’s/guardian’s employment or place of residence. 5. All students must fill out the Student Residency Questionnaire found in the enrollment packet every school year because the status of the homeless situation may change. 6. The Student Residency Questionnaires of students who do not meet any homeless situations must be kept in a binder by the parent educator or social worker for 2 school years and then sent for retention to Admissions & Records. If a student faces a homeless situation during the year, then pull out the Student Residency Questionnaire from the binder and update it. Make sure to give the form to the PEIMS administrator to ensure proper coding is entered in SkyWeb and information is placed inside the yellow At-risk folder in the permanent record. 7. Questions regarding services to be provided to students facing homeless situations may be directed to the Executive Director for Student Services at 323-2688.
