HILL EMERGENCY RESPONSE PLAN EXHIBIT 3 Minor Injury/Illness – Reporting Protocol (Level 1) Protocol Purpose. Reporting protocol for reporting a minor injury and starting the investigation process Scope. Minor Injury/Illness - Any employee injury resulting out of the course and scope of employment. Minor Injury Reporting Instructions –Current reporting protocol.  Call MEDCOR at - 800.775.5866  Inform MEDCOR that the injured is a Hill Mechanical Corp., Hill Mechanical Service, Aero Testing and Balancing or Hill Fire Protection.  Explain to the triage nurse the extent of the injury (MEDCOR also has interpreters).  Employee might be guided into (self-care) first aid, or may be referred to an offsite Designated Medical Facility for further evaluation.  If first aid (self-care) is recommended the triage nurse will fax or send via email follow-up self-care instructions.  After the injury has been stabilized have someone contact your Hill Group Supervisor.  The supervisor/OU Manager shall report the incident to the Safety Director or Risk Manager.  Safety Director and/or Risk Manager will proceed with an investigation if necessary.  Follow normal claims handling procedures. Definition - Assumptions.     

The injury/Illness is not serious or a fatality. Property is in operable condition. Medical care is needed and a medical facility is accessible. Incident was reported in a timely manner. Vehicle damage is minimal and vehicles are “drivable.”

Occurrence of an Injury.

Upon the occurrence of all injuries employees shall provide notice to their immediate supervisor. If their immediate supervisor is not available the injured employee shall provide notice to another supervisor on the job site. If no Supervisor(s) are available, contact MEDCOR first following up with their immediate Supervisor upon their return.



Supervisor. o Upon notice, the Supervisor shall perform a severity assessment of the injury. o Perform the steps as noted by the initial instruction section referenced-above. o Complete Supervisors Incident Report. o Notify appropriate CM, GC, Owners Representative or owner.



Medical Care Necessary. o Should it be determined that medical care beyond First Aid is necessary the injured employee shall be accompanied to the medical facility by a representative of the Company.  A representative of Hill shall escort the injured employee to the preferred medical provider.  While in transit to the preferred medical provider the Safety Director shall be contacted with notice of the incident. The Safety Director may meet and relieve the representative at the medical facility if this is possible. o The purpose of being present at the medical facility is as follows  To assist with providing the necessary Workers’ Compensation information to register the injured employee at the medical facility.  To assist with any questions that the physician may have of employer.  Communicate to the physician that temporary modified duty is available.  To report your knowledge if any of the incident to the medical representative.  To Request the physical limitation of the injured employee to ensure that appropriate temporary modified duty is made available to the injured employee.



Investigation o Upon notice of the injury the supervisor shall take photograph of the scene of the incident. o Obtain from the injured employee  An account of the task being performed prior to the injury.  How the injury occurred?  Who witnessed the injury?  What hazard(s) caused the injury?  How the task should have been performed to prevent the injury? o Supervisor to report their findings to the Safety Director. If necessary, the Safety Director shall proceed with any additional investigation.



Claims Handling o Human Resources shall upload the incident onto Hill’s PENTA Incident Reporting Module within 24-hours of notice of the incident. o The Human Resources Manager shall complete and distribute the necessary forms to ACIG. o The Human Resources Manager shall monitor and provide direction to manage the Workers’ Compensation case.
