Permanent Supportive Housing ◼

Described as a three legged stool that includes: ◼

Affordable apartments-Windham & Windsor Housing Trust



Subsidy-Vermont State Housing Authority



Supportive services-Groundworks Collaborative & Health Care and Rehabilitative Services



An evidenced based model following a Housing First approach, targeted at those struggling with chronic homelessness



Services are voluntary and designed to build independent living and tenancy skills, and connect people with community based services

An Exciting Collaboration ◼

In 2014, representatives from Groundworks and Windham & Windsor Housing Trust visited a PSH site in Connecticut



In 2016, representatives from several local agencies attend the Corporation for Supportive Housing’s national conference in Chicago



After Chicago, a small group formed to begin planning for what was to become Great River Terrace, including staff from GWC, WWHT, and HCRS



Over the past two years, the group worked through group dynamics, created plans for prioritization, service delivery, and hired staff together



Eventually the planning group became an advisory committee, and the service team located at GRT took over the daily operations as we began lease ups in July

Mission Statement ◼

The mission of Great River Terrace is to provide access to affordable apartments regardless of a person’s income, background, or disability.



Residents will be actively involved in the design of supportive services, which will reflect the resident’s needs and goals.



Services will be voluntary, accessible and flexible, with a focus on self-determination and fostering independence.



Great River Terrace will promote respect, the ability of residents to feel safe and comfortable in their homes, and support for individual choice and empowerment.

Tenant Prioritization ◼

Tenants were prioritized based upon a completed Coordinated Entry (CE) Assessment, and the following criteria: ◼

11 units for the chronically homeless will be prioritized through CE based upon meeting the HUD definition of chronic homelessness indicating long term service needs and complex needs score 2 units for the Department of Corrections will be direct referrals from the local Probation & Parole office 4 units for those at risk of homelessness will be prioritized through CE using the medium term service needs definition, and date placed on the list 4 units for those at risk of homeless will be prioritized through CE using the short term service needs definition, and date placed on the list 1 resident key holder unit

Service Planning ◼

Groundworks provided a full-time Case Manager available on-site, thanks to a major gift, and grant dollars from the Department of Corrections



HCRS provided a part-time Case Manager available on-site two days per week, funded through remaining ADAP funds, with the goal of increasing engagement to move towards the ability to bill



WWHT provided an on-site Housing Support Coordinator to manage the daily operations, support engagement from tenants, and to liaise between tenants, on-site staff, and property management



We created a host of service planning tools, with support from the Corporation for Supportive Housing’s staff and tools ◼ ◼ ◼ ◼
