SIUE SCHOOL OF PHARMACY Grade Point Average Calculation Form Instructions
The Southern Illinois University Edwardsville School of Pharmacy requires a minimum Grade Point Average (GPA) of 2.5 to be considered for admission. The Admissions Committee will consider your cumulative GPA for all post-secondary courses attempted. This will be calculated based upon receipt of official transcripts from all the regionally accredited colleges and universities you have attended. You must complete the Cumulative Grade Point Average Calculation Form, the Pre-Pharmacy Courses Grade Point Average Calculation Form, and the Pre-Pharmacy Science and Math Courses Grade Point Average Calculation Form. After completion, please mail all forms along with two complete Recommendation Forms and signed/dated Accreditation Disclosure Statement to: School of Pharmacy Admissions c/o SIUE Service Center, Box 1080; Edwardsville, IL 62026-1080. The deadline for receipt of all of these forms is December 1, 2007. To calculate your grade point average, use the four-point grading scale: A+ A A-

1. Multiply the number of hours you earned in each course by the number of points for each grade (this will equal your quality points). 2. Total the number of quality points. 3. Total the number of hours. 4. Divide the total number of quality points by the total number of hours. For each course you have completed, include the following: the exact course title and number, the college/university where you took the course, the term during which you took the course (e.g. Fall 2007, Spring 2008), the semester hours you earned for the course, the grade you earned, and the number of quality points you earned. If you have not completed some courses in the pre-pharmacy curriculum, list them on the form as well and indicate when you plan to take the courses, which university or college you plan to take them at, and the exact course names and numbers. Do not include the courses you have not yet completed when you perform the GPA calculation. If you repeat a course in the Pre-Pharmacy curriculum, the grade from the first repeated course should be used in the calculation of the GPA. Grades for second and all subsequent repeats of the same course must be used in the calculation of the GPA. The Admissions Committee will check your calculations for accuracy. However, it is important that your documentation of courses and calculation of the GPA is correct. Check your work carefully.
