Important Notes: 1. Please send in entire reservation form (do not cut strips). Extra forms will be available at group meeting or you can pick up form in the GO office. Must submit a form to attend. All prices are listed are per person. 2. If you register to attend an event on the registration form please note that all events (EFFECTIVE IMMEDIATELY) will be NONREFUNDABLE due to our current budget cutbacks which include the lunch bunch ($5) NONREFUNDABLE transportation fee. 3. If an event requires payment by check please submit separate checks for events unless otherwise stated and SIGN your check. 4. When attending “LUNCH BUNCH” please be prepared that this event is an “ALL DAY” event with side TOUR and will not return back to the hospital until event is over.
