If you choose to mail in your promotion request: • Complete ALL information on the promotion form—including the product model number and serial number. Incomplete forms will not be processed. Mark the model purchased on this form. • Include the retail invoice or sales slip (photocopy acceptable) which shows the model number, name of store where the appliance was purchased, date of purchase and purchase price. 3. Promotion submission must be submitted online or postmarked no later than March 3, 2018. Late submissions will not be accepted.
