Delivery/Packaging Slip (Production Record and Temperature Record) Created to ensure snacks and meals served to Head Start children meet USDA Guidelines. Provides a check balance system for staff and vendors. Fulfill USDA requirements for maintaining production records. Created to ensure compliance with Head Start Performance Standards, to identify concerns or strengths with food delivery and holding procedures, and to monitor equipment as well as to ensure safe food handling.

Specific Directions:

1. Vendors will provide their own Delivery/Packaging slip daily for snacks and meals they provide to assure compliance with USDA requirements for feeding 3 to 5 year olds. 2. Vendors will record on Delivery/Packaging Slip temperature of hot food upon leaving vendor kitchen. 3. Teacher/Teacher Assistant will verify accurate amounts of food are sent and appropriate for special diets, if any. 4. Teacher/Teacher Assistant uses food thermometers to check temperatures of all hot and cold foods on a daily basis. 5. Teacher/Teacher Assistants will complete the Delivery/Packaging slip with appropriate information to include delivery time and temperature upon arrival. 6. Teacher/Teacher Assistants will make a copy of the Delivery/Packaging slip. Will send original back to the vendor and keep copy to maintain on file. 7. Teacher/Teacher Assistant will document changes to food served on the menu for all meals and snacks and post where parents can see it. This also includes documentation of specific fruits and vegetables. 8. At the end of the month, Teacher/Teacher Assistants will attach the month’s Delivery/Packaging slip to a menu with changes and turn into the central office. 9. Nutrition Consultant will monitor Delivery/Packaging slip during USDA site visits and on a monthly basis. 10. Concerns are addressed immediately. 11. Central Office for Nutrition Consultant.
