David G. Smith Memorial Award Nomination Form What is the basis for the award? Established through the generosity of Dr. Steve Smith, Dean of Science at Saint Mary’s University, and Dr. Isabel Fearon. The award is made in memory of Steve’s brother, David G. Smith and was inspired by a desire to actively reduce the stigma associated with mental illness. This award is intended to encourage people to share their stories and experiences with mental illness and to celebrate the strength and resilience of people living with mental illness. What is the value of the award? This is a $1000 non-renewable scholarship. Although people who have received the award in one year can re-apply the next year, preference will be given to new applicants. Who can apply? Any returning Saint Mary’s University undergraduate or graduate student studying on a full-time or part-time basis whose experiences and endeavors help to reduce the stigma associated with mental illness. This can be either through first-hand experiences that provide an example of the strength and resilience of people living with a mental illness, or by supporting and celebrating others who are living with a mental illness. All information provided is private and confidential and will only be reviewed by the members of the selection committee. How to apply? Students must nominate themselves. Students will still need to provide a short statement of reflection to share their stories and experiences with mental illness, to illustrate efforts to strengthen resilience of people living with mental illness, and/or increase mental health awareness.

A selection committee consisting of: - the Senior Director, Student Affairs and Services (Chair), - a representative from Counselling or Health Services - a representative who is a faculty member - an additional faculty or staff member may be added at the Chair’s request Deadline for submission is Friday, December 7, 2018.
