Pastoral Institute Diocese of Brooklyn Lay Ministry Program APPLICATION PROCESS CHECKLIST Prior to beginning the application process, please review the following materials: Program Overview Class Locations and Schedules for 2013-2014 including Orientation Session, Learning Skills Workshop and Days of Reflection Print and complete Application Form including page 5. Please note that we must have an email address for every person. Also, please attach a recent photo. Print and give a Recommendation Form to the two persons whom you list as references on your Application Form (these forms will be returned directly to the Pastoral Institute). Print and review the Interview Schedule and Class Selection. Please check all times that you are available for the interview. At the bottom of the page, write #1 before your first choice for a class location and #2 before your second choice. Print and complete the Authorization and Disclosure Form for a background check that will be conducted by an outside agency. It will include a verification of your social security number, a criminal file check, and a state sexual-offender search. There is no search on one’s finances. Mail the following to the Pastoral Institute on or before July 31, 2013: Application Form including page 5 and a photo

A $35.00 check for a non-refundable application fee must accompany this Application Form. If your Parish is covering this cost, please get the check from them and include it with your application form. Checks should be made payable to the DIOCESE OF BROOKLYN.
Upon receipt of the above documents, you will receive a letter from the Pastoral Institute which will indicate the date, time, and location for your interview.
