The child’s parents.(1) Within 30 days of the child’s birth.(1) The child’s next-of-kin(1) or legal guardian.(1) Notice is given after an enquiry(1) has been undertaken in terms of the Children’s Act 38 of 2005 by the social worker(1) involved in the enquiry. The following documents may be submitted: 5.1 Proof of birth form attested to by a medical practitioner who attended to the child’s birth or examined the child or his or her mother after the birth(1) 5.2 An affidavit by a South African citizen who witnessed the birth if the birth did not take place at a health institution(1) 5.3 A palm, foot or fingerprint of the child(1) 5.4 Fingerprints of the parent(s)(1) 5.5 Certified copies of the identity document, passport, visa of permit of the parent(s), next-of-kin or legal guardian(1) All of the above documents must be submitted (5 marks) plus a form(1) setting out the reasons why notice is being given late.(1) No,(1) a child may not be registered as CJ Sibanyoni as a child may not be registered unless a forename and a surname have been assigned to the child.(1)
