Choose the space you would like to update

Select “Door Sign”

Input the PI’s name and office phone number as the first emergency response contact

Please note: graduate students cannot be listed as a secondary emergency response contact.

Input your emergency phone number. Your emergency number will not be posted on the door, it is only for emergency responders use in case of an incident that occurs in your lab off hours.

Second contact MUST be an employee of the university that has knowledge of your research who is able to help coordinate emergency response.  Examples of this can include Lab Manager, Postdocs, Department Safety Officers, etc.

3

You must select the lab hazards for this particular space. Please note you can auto-populate the space hazards to be the same as your lab, but this should represent what hazards are physically present in this space.

For example, a lab group may do work with a 3B laser, but if the laser is not stored/used in this specific space, it should not be listed as a “space hazard”.

4

Fill out your NFPA 704 Diamond information and Personal Protective Equipment/Precautions. In the NFPA, diamond there will be the highest hazard rating for each category (flammability, health, and reactivity) found in your inventory. This can be found on each chemical’s safety data sheet (SDS). Please review your inventory and update as needed.

For further information and common chemical ratings please review the NFPA Hazard Rating System page on the EH&S website located at: https://www.northeastern.edu/ehs/ehsprograms/laboratory-safety/generalinformation/nfpa-hazard-rating-system/

Our General lab PPE requirements are FR Lab Coats, Safety Glasses, Closed Toe Shoes, Gloves, and No Food/Drink. Depending on your research, this may include further PPE requirements.

Once completing the Digital Door Sign, please select “Build Door Sign”.

5

Once you complete the Digital Door Sign, please select “Build Door Sign”. From there it will ask you to “Select Door Sign Template”. Please select “Default GHS” as the template. If you are submitting a sign request for a maker space, with no chemicals select “Default GHS- No NFPA”.

6

Here is where we will create the physical door sign that will be posted on the entrance of your laboratory door.

Please select either NUBoston Campus or NUBurlington/Nahant Campus depending on where your lab is located.

Select landscape layout.

Remove your personal phone number from this section.

For Header choose “Authorized Personnel Only”.

7

Out of the “Area Hazard” options, choose the five highest hazards in this space from your digital door sign. You will not be allowed to choose anymore so please make sure there are only five listed and they correctly identify the greatest hazards in the lab (ex. Acute Toxicity should be identified over irritant).

Select six PPE/Precautions to list on the door. They should be the same as posted on the digital copy. If there are any special notes you would like to add for entrance procedures/precautions please enter that in the “Entrance Procedures/Facility Instructions” box. This will be posted on the door.

8

Once you click submit you have completed your door sign! EH&S will be able to track newly updated signs and will print and deliver as they are updated. This finished product will look something like this:

9

Lastly, to assign the capability of updating the door signs to other members in your lab, you must edit their “optional access in BioRAFT”. This done by: 1. Selecting the members tab from your lab homepage.

2. Selecting edit next to the lab member you would like to assign the capability

3. Checking the “Create Digital Door Signs” and “Generate Door Signs” boxes under “Optional Access in BioRAFT”.
