ON-CALL ARCHITECUTRAL & ENGINEERING (A&E) SERVICES ADDENDUM Number 4 dated JANUARY 12, 2016 The Alameda-Contra Costa Transit District herewith issues this Addendum No. 4 to the above referenced Documents. Except as modified below, all other terms and conditions shall remain in effect. Strikethrough texts are words deleted from original RFQ text and bold/italicized/underlined/red/ text are changes to original RFQ text. 1.

QUESTION Addendum No 3, Question 12 states the following: “If we have sub-consultants on our team does each sub-consultant need to provide an SOQ and SF330 Part I and II?” RESPONSE Yes, each sub-consultant needs to provide an SOW SOQ and SF330 Part 1 and Part 11.

CHANGE TO SUBMITTAL DATE: Because the District has requested the above information from sub-consultants the District has extended the submittal date of all SOQ’s to January 29, 2016 as described below. This will allow primes to better prepare their submittals. 2.

QUESTION Selection Criteria – For the selection criteria, can we provide this in an additional section, or would you like our response to be within SF 330, Section H? RESPONSE Please provide this information within SF 330, Section H.

QUESTION Could you please clarify for a question regarding 5. Annual Gross Receipts? Should we include annual gross receipts from the most recent 3 years with the RFQ? Or does this mean that we just need to have these documents ready in the event that the Procurement & Materials Director /Chief Financial Officer may request them?

However, documentation should be readily available in the event the Procurement & Materials Director/Chief Financial Officer request proof. QUALIFICATION SUBMITTAL DATE, TIME AND LOCATION Qualification packages must be received by 2:00 pm on January 15, 2016 a.m. 2:00 pm on January 29, 2016 p.m. at: AC Transit Purchasing Department Attn: Nocoasha L. Henry, Contract Specialist 1600 Franklin Street, 8th Floor. Oakland, CA 94612 SOQ’s may be mailed or hand delivered. SOQ responses received after the date, time or at any other location will not be accepted. No facsimile or e-mail transmissions of proposals will be accepted. All SOQ’s should be clearly marked 2016-1349 On-Call Architectural & Engineering. No other methods of delivery shall be accepted such as telephone, facsimile, e-mail or telegraphic. Such submittals received shall be returned unopened and shall not be considered.
